Reimbursement Form

The reimbursement form below is for volunteers who have purchased supplies that will be reimbursed by the church.

Before making any purchase, please check in with the church office first (585-924-2621 or email). Also note that the church’s tax-exempt status can only be used if you purchase with church funds directly. If you are purchasing using your own funds to be reimbursed, you will need to pay sales tax.
